Moves, Adds, Changes for Phones
and Network Jacks
Moving, adding or changing phone lines/sets and network jacks require submission of a TEO to the Office of Information Technology (OIT). You can fill out and submit a TEO online, or you can print out a TEO (in pdf format) and fax it to OIT.
